Volunteer Under 18 - Parent/Guardian Consent

As a volunteer with Ventura Land Trust (VLT), I agree that:

1. I will uphold and mission and standards of VLT (see website for details).
2. My services are donated without contemplation of any compensation, benefit or future employment and are being provided solely for humanitarian and charitable reasons.
3. I will be punctual and conscientious, conduct myself with dignity, courtesy, and consideration for others, and will do my best to fulfill commitments that I make to VLT by completing all assignments which I accept in a timely and professional manner.
4. I will not sell or attempt to sell any goods, request contributions (charitable or otherwise), or distribute or solicit any person to sign any political petitions, except to the extent that I receive express written authorization, in advance, from VLT’s Volunteer Coordinator or a Board Trustee.
5. I will promptly raise concerns and attempt to resolve any problems related to my volunteer activities with my Supervisor/Project Manager. If those attempts are unsuccessful, I will promptly raise concerns and attempt to resolve any problems with VLT’s Volunteer Coordinator. If those efforts are not successful, I will attempt to resolve any problems with VLT’s President or Executive Director.
6. I agree to give my Project Manager/Supervisor and/or Volunteer Coordinator timely notice if I decide to resign as a volunteer for VLT, so that any transition to a new person to fill my job can be done with little impact to the job.
7. I understand that VLT reserves the right to terminate my volunteer status at any time as a result of (a) my failure to comply with VLT policies, rules or regulations, including any actions inconsistent with VLT’s status as a tax exempt charitable organization; (b) absences without prior notification; (c) unsatisfactory attitude or work; (d) use of any confidential information regarding VLT in an inappropriate manner; (e) any actions on my part that could be construed as sexual harassment; or (f) other circumstances which, in the judgement of my Supervisor/Project Manager, VLT’s Volunteer Coordinator, or VLT’s Board of Trustees, would make my continued service as a volunteer contrary to the best interests of VLT.
8. I understand that I can and should decline to provide any services which I believe will result in any damage to property or any physical injury or other harm to me or any other person. I agree to assume the full risk of any loss, damage, or destruction to property or any injury or illness that may occur as a result from providing my volunteer services. I waive, to the fullest extent permitted by law, all claims and demands which may arise or result in the course of providing my volunteer services, and I release VLT and its employees, officers, trustees and volunteers from all claims, demands, losses and liabilities of every kind and nature, including claims for personal injury and loss or damage to property arising out of or in connection with my services as a volunteer.
9. I give permission to VLT to use photographs or video footage taken of me while participating in VLT activities.
10. I have read and understand each of the above conditions and I agree to be bound by them.
